How to fill in the proxy form in favour of the change of RE

Here is an example of a completed proxy form filled in favour of the change of Responsible Entity

This example assumes you will not be attending the meeting itself but will submit your vote by post, fax or email and so will appoint the Chairperson of the meeting as your proxy. The example assumes you will vote in favour of the change of RE.

Written instructions are also included on pages 14 – 15 of the Notice of Meeting if you wish to appoint a proxy other than the Chairperson, or attend the meeting in person.
